HIPAA Security Rule Overview

The HIPAA Security Rule establishes national standards for protecting electronic Protected Health Information (ePHI). Healthcare organizations must implement administrative, physical, and technical safeguards to ensure confidentiality, integrity, and availability of ePHI.

Technical Safeguards (164.312)

Access Control (164.312(a))

Unique User Identification: Every user must have a unique identifier for tracking access to ePHI.

Emergency Access Procedures: Documented procedures for obtaining necessary ePHI during emergencies.

Automatic Logoff: Sessions must terminate after a defined period of inactivity.

Encryption and Decryption: ePHI must be encrypted when stored or transmitted.

Audit Controls (164.312(b))

Implement mechanisms to record and examine activity in systems containing ePHI:

  • Login/logout events
  • Data access and modification
  • Failed authentication attempts
  • Administrative actions

Integrity Controls (164.312(c))

Mechanisms to authenticate and protect ePHI from improper modification:

  • Checksums and digital signatures
  • Version control for documents
  • Database integrity constraints

Transmission Security (164.312(e))

Protect ePHI during electronic transmission:

  • TLS 1.2+ for all data in transit
  • Encrypted email for ePHI communication
  • VPN for remote access to ePHI systems

Implementation Best Practices

  • Deploy centralized access management with MFA
  • Implement automated audit log collection and retention (6 years minimum)
  • Use encryption at rest (AES-256) and in transit (TLS 1.3)
  • Conduct regular risk assessments as required by 164.308(a)(1)
  • Train all workforce members on ePHI handling procedures

SuperClue HIPAA Module

SuperClue's HIPAA framework scan runs 12 checks named after Security Rule sections, such as 164.312(a) access control and 164.312(e) transmission security. It checks your external website and asks for evidence for the rest. The report helps you prepare; it is not an HHS audit.
